Overview: The High Five Rewards Program (the "Program") is a service of BayCare Health System and its hospitals, facilities, affiliates, and/or subsidiary companies ("BayCare"). This Program is provided for all eligible team members of BayCare. All BayCare team members using this website agree that their access to and use of this website are subject to the terms and conditions set forth herein and all applicable laws. Any access is undertaken at the team member's own risk.

Eligibility: Eligibility for the Program is limited to all current BayCare team members. Points earned in the Program are forfeited upon the effective date of any voluntary/involuntary termination. No credit, carryover, transfer or redemption of points is available after eligibility is severed. All questions or disputes regarding eligibility for the Program, the eligibility of rewards, the availability of rewards or points, and/or the terms and conditions of the Program will be resolved by BayCare in its sole discretion.

Change: BayCare reserves the right, without cause and without prior notice, to discontinue, suspend, and/or change the Program at any time. BayCare reserves the right to change these terms and conditions at any time without prior notice. It is your obligation to check the website and these terms and conditions from time-to-time for any changes. Your continued use of the Program and this website shall constitute your ongoing acceptance of the then-current terms of the Program and the then-current terms and conditions.

Participant Enrollment: Each eligible team member is automatically enrolled in the Program and is assigned a user login, used to gain access to the Program web-site. Upon initial login, team members are required to change their password.

Redemption and Deduction of Points:

  • Points earned in the program are forfeited upon the effective date of any voluntary/involuntary termination.
  • Reward points may only be used to redeem selections from the online catalog. Points are not redeemable for cash.
  • The reward points you accrue do not constitute your property. You cannot transfer reward points to another team member. Reward Points have no monetary value except to the extent specifically provided for by a reward.
  • Reward points will expire five (5) years after being awarded – unless your eligibility is severed prior to the expiration of such five (5) year period. It is the responsibility of each eligible team member to redeem their points during the redemption period.

Claiming a Reward:

  • BayCare reserves the right to modify the merchandise available in the online catalog at any time and without notice.
  • When you have obtained sufficient points, you may redeem an item(s) from those described in the online catalog.
  • Reward selections are subject to availability, and substitutions may be made based on availability or market conditions.

Statements of BayCare Reward Points Earned: This web-site provides eligible team members with details of transactions, as well as Program reward point balances.

Termination of Program: BayCare reserves the right to suspend or terminate the Program at any time without cause and without any prior notice to you. If the Program is suspended or terminated, a final redemption period will be announced for any unredeemed reward points.

Participant Program Tracking: It is the responsibility of each eligible team member to use their awarded points to redeem items from the online catalog. Awarded points can only be redeemed for items presented in the online catalog.

Promotional Events: From time to time, BayCare at its discretion, may sponsor events intended to promote participation in the Program. These events may be terminated at any time, without cause or notice.

Taxes: Reward selections redeemed under the Program are subject to applicable federal, state and/or local taxes. Payment of taxes resulting from these awards is the sole responsibility of the participating team member and not BayCare. BayCare will report earnings under this Program in accordance with federal, state and local regulations.

Shipping and Handling: Reward selections redeemed under the Program are subject to shipping and handling charges. The shipping and handling charges are included in the point value associated with each item in the online catalog and are represented in the overall value of the item.

Privacy Notice: This web-site is housed and operated by Online-Rewards on behalf of BayCare. The information collected on this web site will be used by BayCare in connection with the Program. Online-Rewards will not provide your information to any other parties.

Waiver and Release: As part of your participation in the Program, you agree to forever release, indemnify, and hold harmless BayCare from any and all liabilities, damages, claims, costs, and/or expenses arising from, or in any way related to, this website, the Program, and/or any items redeemed under the Program.

Confidentiality: If you use this web-site, you are responsible for maintaining the confidentiality of your account and password and for restricting access to your computer, and you agree to accept responsibility for all activities that occur under your account or password.

Program Point Expiration: All unused points older than 5 years will expire from your account the following month and be automatically forfeited. Example - points awarded January 3, 2012 will expire and be removed from your account February 1, 2017.
